Abstract Introduction Early mobilization is an alternative used in the Intensive Care Unit (ICU) to reduce the effects of immobility. The Intensive Care Unit Mobility Score (IMS) is applied to assess mobility status. Objective To determine the functional level of ICU patients submitted to cardiac surgery using the IMS scale. Methods This is an analytical observational study carried out with adult ICU patients submitted to cardiac surgery. Data on the use of vasoactive drugs, functional level through IMS, duration of mechanical ventilation and length of ICU stay were collected from the physiotherapy routine form and recorded on a specific instrument developed for the study. The data obtained from the IMS scale score on the postoperative days in the ICU were used to classify the patients' mobility during the hospitalization period. Descriptive statistics were used to present the data. Results A total of 69 patients were evaluated, 43% of whom were men, and all had an SMI of 0 on the first postoperative day. Classification on the scale increased over the course of hospitalization (IMS between 7 and 10), despite the use of vasoactive drugs in 54.6% of the individuals. Conclusion Patients submitted to cardiac surgery admitted to the ICU had moderate-to-high mobility levels throughout their stay and at discharge from the ICU.


Resumo Introdução A mobilização precoce é uma alternativa utilizada em Unidades de Terapia Intensiva (UTIs) na tentativa de reduzir os efeitos decorrentes do imobilis-mo. A escala de mobilidade em UTIs ou Intensive Care Unit Mobility Score (IMS) é aplicada para avaliar o nível de mobilidade. Objetivo Verificar por meio da escala IMS o nível de funcionalidade de pacientes submetidos à cirurgia cardíaca internados em uma UTI. Métodos Trata-se de um estudo observacional analítico realizado em UTI com pacientes adultos submetidos à cirurgia car-díaca. Foram coletados da ficha de rotina da fisioterapia informações quanto ao uso de drogas vasoativas, nível funcional por meio da IMS, tempo de ventilação mecânica e de internação na UTI, e registrados em instrumento específico desenvolvido para o estudo. Os dados obtidos do escore da escala IMS nos dias de pós-operatório na UTI foram utilizados para classificar a mobilidade dos pacientes durante o período de internação. Utilizou-se estatística descritiva para a apresentação dos dados. Resultados Foram avaliados 69 pacientes, 43% eram do sexo masculino e todos apresentavam IMS 0 no primeiro dia de pós-operatório. A classificação na escala aumentou com o decorrer do período de internação (IMS entre 7 e 10), apesar do uso de drogas vasoativas em 54,6%. Conclusão Os pacientes submetidos à cirurgia cardíaca internados na UTI apresentaram níveis de mobilidade de moderado a alto ao longo da internação e na alta da UTI.

ABSTRACT Introduction: Early mobilization of patients in the postoperative period of cardiac surgery who are hospitalized in the intensive care unit (ICU) is a practice that has a positive impact. Methods: This is a systematic review of studies published until September 2020 in the Medical Literature Analysis and Retrieval System Online (or MEDLINE®), Embase, Physiotherapy Evidence Database (or PEDro), Scientific Electronic Library Online (or SciELO), and Latin American and Caribbean Health Sciences Literature (or LILACS) databases. Randomized clinical trials describing mobilization protocols performed early in ICU patients after cardiac surgery were included. Results: According to the eligibility criteria, only 14 of the 1,128 articles found were included in the analysis. Early mobilization protocols were initiated in the immediate postoperative period or first postoperative day. The resources and technics used were progressive mobilization, cycle ergometer, early bed activities, walking protocols, resistance exercise, and virtual reality. Intensity of the mobilization activities was determined using the Borg scale and heart rate. Conclusion: Early mobilization protocols are generalist (not individual), and low-intensity exercises are used, through progressive mobilization, with two daily physical therapy sessions, during 10 to 30 minutes.

ABSTRACT

Objective: To evaluate a protocol for acceptance and commitment therapy-based behavioral intervention for insomnia (ACT-BBI-I) in adults compared to cognitive behavioral therapy for insomnia (CBT-I). Methods: Forty-five adults with chronic insomnia were randomized to ACT-BBI-I or CBT-I. Both interventions were performed in six weekly group sessions. The common treatment elements in both protocols included stimulus control and sleep restriction. CBT-I is focused on the cognitive restructuring of maladaptive beliefs about sleep and the daytime effects of insomnia. ACT-BBI-I focuses on therapeutic processes of acceptance, availability, values, defusion, and commitment. The results were evaluated through the following instruments: a sleep diary, the Insomnia Severity Index, the Epworth Sleepiness Scale, the Hospital Anxiety and Depression Scale, the Acceptance and Action Questionnaire-II, and the Dysfunctional Beliefs and Attitudes about Sleep scale. Results: Both interventions had a significant positive impact on sleep patterns, insomnia, anxiety, beliefs about sleep, and psychological flexibility. All improvement was maintained at the 6-month follow-up. Conclusion: The results suggest that integrating principles of ACT with behavioral techniques may be useful for treating insomnia. Further research should identify whether the principles of ACT result in added effectiveness compared to behavioral components alone. Clinical trial registration: RBR-7nc5wq

Abstract Introduction: Coronary artery bypass grafting (CABG) is associated with reduced ventilatory muscle strength and consequent worsening of functional capacity (FC). Inspiratory Muscle Training (IMT) can be indicated, but there is still a lack of knowledge about the use of the anaerobic threshold (AT) as a basis for prescription. The objective of this study is to evaluate if IMT based on AT modifies FC and inspiratory muscle strength of patients submitted to CABG. Methods: This is a clinical trial. On the first postoperative day, the patients were divided into two groups: the conventional group (IMT-C), which performed IMT based on 40% of maximal inspiratory pressure (MIP), and the IMT-AT group, which performed IMT based on AT. All patients underwent preoperative and postoperative assessment of MIP and performed a six-minute walk test (6MWT). Results: Forty-two patients were evaluated, 21 in each group. Their mean age was 61.4±10 years and 27 (64%) of them were male. There was a reduction of inspiratory muscle strength with a delta of 23±13 cmH2O in the IMT-C group vs. 11±10 cmH2O in the IMT-AT group (P<0.01) and of the walking distance with a delta of 94±34 meters in the IMT-C group vs. 57±30 meters in the IMT-AT group (P=0.04). Conclusion: IMT based on AT minimized the loss of FC and inspiratory muscle strength of patients submitted to CABG.

ABSTRACT

ABSTRACT Pterygium pathogenesis has been mainly asso ciated with UV light exposure; however, this association remains quite controversial. The complete mechanism of pterygium also remains to be clarified. Factors such as inflammation, viral infection, oxidative stress, DNA methylation, inflammatory mediators, extracellular matrix modulators, apoptotic and oncogenic proteins, loss of heterozygosity, microsatellite instability, lymphangiogenesis, epithelial-mesenchymal cell transition, and alterations in cholesterol metabolism have been identified as causes. Several studies aimed to clarify the molecular mechanisms underlying the growth and proliferation of pterygium. Understanding its molecular basis provides new potential therapeutic targets for its prevention and treatment. A comprehensive search of the databases, namely, MedLine, EMBASE, and LILACS, was conducted with the following key words: pterygium, epidemiology, pathogenesis, biomarkers, and review. This review describes the epidemiology, clinical presentation, and current investigation of biological mediators involved in pterygium development.


RESUMO A patogênese do pterígio tem sido relacionada, prin cipalmente, à exposição à luz ultravioleta, mas esta asso ciação permanece bastante controversa. O mecanismo completo do pte rígio também permanece por esclarecer. Fatores como inflamação, infecção viral, estresse oxidativo, metilação do DNA, mediadores inflamatórios, moduladores de matriz extracelular, proteínas apoptóticas e oncogênicas, perda de heterozigose, instabilidade de microssatélites, linfangiogênese, transição celular epitelial-mesenquimal e alterações no metabolismo do colesterol tem sido identificados como causas. Diversos estudos visam esclarecer os mecanismos moleculares subjacentes ao crescimento e proliferação do pterígio. Entender sua base mo lecular fornece novos alvos terapêuticos potenciais para sua prevenção e tratamento. Uma busca abrangente nas bases de dados, a saber, MedLine, EMBASE e LILACS, foi realizada com as seguintes palavras-chave: pterígio; epidemiologia; patogênese; biomarcadores e revisão. Esta revisão descreve a epidemiologia, apresentação clínica e a atual investigação de mediadores biológicos envolvidos no desenvolvimento do pterígio.

ABSTRACT

O modelo assistencial contemporâneo tem como foco principal o paciente em toda sua complexidade. A Estratégia de Saúde da Família (ESF) faz parte da Rede de Saúde e preza pela visão holística no trabalho coletivo e, assim, a Política Nacional de Humanização (PNH) torna-se uma ferramenta de estruturação do trabalho na Saúde Pública. Neste sentido, o presente trabalho tem como objetivo identificar as principais mudanças que a PNH influenciou na ESF e na Rede de Saúde, a fim de identificar as contribuições prestadas. Foi realizada Revisão Bibliográfica de Literatura, por meio da análise de artigos da Biblioteca Virtual em Saúde (BVS) e documentos oficiais do Portal do Ministério da Saúde. Para a busca na BVS foram utilizados os descritores "Estratégia de Saúde da Família", "Política Nacional de Humanização" e "Atenção Básica". Foram selecionados 10 artigos pertinentes ao tema do estudo. Os resultados mostraram que sendo a PNH uma nova forma de se pensar em saúde, oferece ferramentas capazes de auxiliar nas ações e na tomada de decisão nos serviços de saúde. Concluiu-se que a ESF e a Rede de Saúde passam por uma continua transformação e que a PNH contribuiu com ferramentas de trabalho, qualificando a atenção prestada ao usuário, bem como proporcionando reflexão ética, política e compartilhada entre gestores, trabalhadores e usuários.


The contemporary model of care focuses primarily on the patient in all its complexity. The Family Health Strategy (ESF) is part of the Health network and price for the holistic view also on collective work and the National Humanization Policy (PNH) is a restructuring working tool in Public Health. In this sense, the present work aims to identify major changes influenced by PNH in the ESF in the health network to identify the contributions made. A Literature Bibliographic Review was performed in the Virtual Health Library (BVS), by analyzing articles, and official documents in the Ministry of Health Portal. To search the BVS were used the key words "Family Health Strategy," "National Humanization Policy" and "Primary Care". There were selected 10 articles relevant to the topic of study. The results showed that the PNH being a new way of thinking about health offers tools to assist in the actions and decision-making in health services. It was concluded that the ESF goes through a continuous transformation and that PNH contributed with working tools, by qualifying the attention given to the user, as well as providing ethical, political and shared reflection among managers, workers and users.

La Estrategia de Salud de la Familia (ESF) forma parte de la Red de Salud y se distingue por la visión holística en el trabajo colectivo y, así, la Política Nacional de Humanización (PNH) se vuelve una herramienta de estructuración del trabajo en la Salud Pública. En este sentido el presente trabajo tiene como objetivo identificar los principales cambios que la PNH influenció en la ESF y en la Red de Salud, a fin de identificar las contribuciones prestadas. Se realizó Revisión Bibliográfica de Literatura, a través del análisis de artículos de la Biblioteca Virtual en Salud (BVS) y documentos oficiales del Portal del Ministerio de Salud. Para la búsqueda en la BVS se utilizaron los descriptores "Estrategia de Salud de la Familia", "Política Nacional de Humanización "y" Atención Básica". Fundamento: As doenças cardiovasculares têm aumentado em todo o mundo nas últimas décadas devido ao aumento na ocorrência de fatores desencadeantes. No período pós-operatório de cirurgia cardiovascular, os pacientes experimentam um declínio funcional, que pode ser potencializado pelo tempo de internação hospitalar. Portanto, é importante avaliar a capacidade funcional destes pacientes. Objetivo: Comparar o tempo de internação hospitalar com a velocidade da marcha em pacientes submetidos à cirurgia cardíaca. Método: Estudo de coorte prospectivo, realizado no Instituto Nobre de Cardiologia (Incardio) da Santa Casa de Misericórdia (Feira de Santana, Bahia). Na alta hospitalar, todos os pacientes foram avaliados com o teste de caminhada de 6 minutos (TC6). O tempo de internação hospitalar no período pós-operatório foi também registrado no momento da avaliação e correlacionado com a velocidade da marcha. Utilizamos o teste de KolmogorovSmirnov para avaliar a suposição de normalidade e a correlação de Spearman para correlacionar a velocidade da marcha com a idade, tempo de hospitalização e com a duração da circulação extracorpórea (CEC) e da ventilação mecânica invasiva (VMI). Todas as conclusões foram baseadas em um nível de significância de 5%. Resultados: Ao todo, 64 pacientes foram incluídos (33 homens [51,5%], idade média 57,2 ± 14,06 anos). A distância média percorrida pelos pacientes foi de 375,8 ± 197,6 metros, a média da velocidade da marcha foi de 0,98 ± 0,53 m/s e a média de permanência hospitalar foi de 8,2 ± 2,3 dias. Foi observada uma fraca correlação entre o tempo de internação hospitalar e a velocidade da marcha (r = 0,27 e p = 0,02). Conclusão: A duração da hospitalização mostrou uma fraca correlação com a velocidade da marcha na alta hospitalar em uma amostra de pacientes submetidos à cirurgia cardíaca


Background: Cardiovascular diseases have been increasing worldwide in recent decades due to the increased occurrence of triggering factors. In the postoperative period of cardiovascular surgery, patients experience a functional decline, which may be potentiated by the length of hospital stay. Therefore, it is important to evaluate these patients' functional capacity. Objective: To compare the length of hospital stay with the gait speed in patients undergoing cardiac surgery Method: Prospective, cohort study carried out at the Instituto Nobre de Cardiologia (Incardio) at Santa Casa de Misericórdia (Feira de Santana, Bahia). Upon discharge, all patients were evaluated with the 6-minute walk test (6MWT). The length of hospital stay in the postoperative period was also recorded at the same time and correlate with the gait speed. We used the Kolmogorov-Smirnov test to evaluate the assumption of normality, and Spearman's correlation to correlate the gait velocity with age, length of hospital stay, and duration of cardiopulmonary bypass (CPB) and invasive mechanical ventilation (IMV). All conclusions were based on a significance level of 5%. Results: In all, 64 patients were included (33 males [51.5%], mean age 57.2 ± 14.06 years). The mean distance walked by the patients was 375.8 ± 197.6 meters, the mean gait speed was 0.98 ± 0.53 m/s, and the mean hospital stay was 8.2 ± 2.3 days. A weak correlation was observed between the length of hospital stay and gait speed (r = 0.27 and p = 0.02). Conclusion: The length of hospital stay correlated weakly with the gait speed upon hospital discharge in a sample of patients undergoing cardiac surgery

Abstract Objective: To apply the InsCor in patients undergoing cardiac surgery in a university hospital in Brazil's northeast. Methods: It is a retrospective, quantitative and analytical study, carried out at the University Hospital of the Federal University of Maranhão. InsCor is a remodeling of two risk score models. It evaluates the prediction of mortality through variables such as gender, age, type of surgery or reoperation, exams, and preoperative events. Data from January to December 2015 were collected, using a Physical Therapy Evaluation Form and medical records. Quantitative variables were expressed as mean and standard deviation and qualitative variables as absolute and relative frequencies. Fisher's exact and Kruskal-Wallis tests were applied, considering significant differences when P value was < 0.05. Calibration was performed by Hosmer-Lemeshow test. Results: One hundred and forty-eight patients were included. Thirty-six percent were female, with mean age of 54.7±15.8 years and mean body mass index (BMI) equal to 25.6 kg/m2. The most frequent surgery was coronary artery bypass grafting (51.3%). According to InsCor, 73.6% of the patients had low risk, 20.3% medium risk, and only 6.1% high risk. In this sample, 11 (7.4%) patients died. The percentage of death in patients classified as low, medium and high risk was 6.3, 7.1% and 11.1%, respectively. Conclusion: InsCor presented easy applicability due to the reduced number of variables analyzed and it showed satisfactory prediction of mortality in this sample of cardiac surgery patients.

RESUMO Este estudo pretendeu verificar se a presença do fisioterapeuta influencia no processo de ventilação mecânica de pacientes submetidos à cirurgia cardíaca não complicada e admitidos em UTI cardiológica no período noturno. Trata-se de estudo documental retrospectivo com pacientes adultos submetidos a cirurgia cardíaca e admitidos na UTI no período noturno nos meses de novembro de 2010 a outubro de 2011, com assistência fisioterapêutica por 12 horas (n=51) e entre novembro de 2011 e outubro de 2012, período com assistência fisioterapêutica por 24 horas (n=43), no Hospital Universitário da Universidade Federal do Maranhão. Para análise estatística, foram utilizados os testes qui-quadrado, t de Student e G, sendo os dados considerados estatisticamente significantes quando p<0,05. A duração da ventilação mecânica foi menor quando ocorreu assistência fisioterapêutica no período noturno (6,7±3,7 horas vs. 8,7±3,1 horas, p=0,02). Nesse mesmo período, o número de pacientes extubados em tempo inferior a seis horas também foi significativamente maior (53,4% vs. 27,4%, p=0,0182), assim como o número de extubações programadas (79% vs. 43,1%, p=0,009). A atuação fisioterapêutica influenciou o processo de ventilação mecânica de pacientes submetidos à cirurgia cardíaca não complicada e admitidos na UTI no período noturno, reduzindo o tempo de ventilação mecânica e aumentando o número de extubações em tempo inferior a seis horas e o número de extubações programadas durante a noite.


RESUMEN En este estudio se propone a verificar si la presencia del fisioterapeuta influencia en el proceso de ventilación mecánica en pacientes sometidos a cirugía cardiaca no complicada e internados en el sector cardiaco de una UCI durante el periodo nocturno. Estudio de carácter documental retrospectivo con pacientes adultos sometidos a cirugía cardiaca e internados en una UCI en el periodo nocturno de noviembre de 2010 hasta octubre de 2011, con cuidados fisioterapéuticos de 12 horas (n=51), y entre noviembre de 2011 hasta octubre de 2012, periodo con cuidados fisioterapéuticos de 24 horas (n=43), en el Hospital Universitario de la Universidade Federal do Maranhão, en Brasil. Para el análisis estadístico, se emplearon las pruebas chi-cuadrado, t de Student y G, siendo los datos considerados significativos estadísticamente cuando p<0,05. La duración de la ventilación mecánica ha sido menor durante el cuidado fisioterapéutico en el periodo nocturno (6,7±3,7 horas vs. 8,7±3,1 horas, p=0,02). En ese mismo periodo, el número de pacientes que extrajeron los tubos en tiempo inferior a seis horas también ha sido significativamente mayor (53,4% vs. 27,4%, p=0,0182), así como la cantidad de retirada programada de los tubos (79% vs. 43,1%, p=0,009). La acción fisioterapéutica influyó en el proceso de ventilación mecánica de los pacientes sometidos a cirugía cardiaca no complicada e internados en la UCI durante el periodo nocturno, les redujo el tiempo de ventilación mecánica y aumentó el número de retirada de los tubos en tiempo inferior a seis horas y la retirada programada durante el periodo nocturno.


ABSTRACT The aim of this study was to verify the influence of physical therapists on the mechanical ventilation process of patients who underwent non-complicated cardiac surgery admitted to the Cardiac ICU overnight. Documentary and retrospective study with adult patients who underwent cardiac surgery admitted to the ICU overnight from November 2010 to October 2011 with physiotherapeutic care for 12 hours (n=51), and from November 2011 to October 2012 with physiotherapeutic care for 24 hours (n=43), at the University Hospital of the Federal University of Maranhão. For statistical analysis, we used Chi-square, Student's t, and G tests; data were considered statistically significant when p<0.05. The duration of the mechanical ventilation was lower when physiotherapeutic care occurred at night (6.7±3.7 h vs. 8.7±3.1 h, p=0.02). In this same period, the number of patients extubated in less than 6 hours was also significantly higher (53.4% vs. 27.4%, p=0.0182), as well as the number of scheduled extubations (79% vs. 43.1%, p=0.009). The physiotherapeutic practice influenced the mechanical ventilation process of patients who underwent non-complicated cardiac surgery admitted to the ICU overnight, reducing the time of mechanical ventilation and increasing the number of extubations in less than 6 hours, as well as the number of extubations scheduled during the night.

ABSTRACT Background Kinesio Taping (KT) is an elastic bandage that aims to improve neuromuscular performance, although there is no consensus as to its benefits. Objective To analyze the immediate and delayed effects of KT on the neuromuscular performance of the femoral quadriceps, on balance, and lower limb function in healthy subjects. Method This is a randomized controlled trial. Thirty-six women with a mean age of 22.2±3.6 years and BMI of 22.5±2.3 Kg/m2 were divided into three groups: control, with ten minutes of rest (control, n=12), application of Kinesio Taping without tension (placebo, n=12) and with tension (KT, n=12) on the quadriceps. The primary outcome was isokinetic performance, while secondary outcomes were the single-hop test, one-footed static balance, and electromyographic activity. The evaluations were carried out in five stages: 1) before application of KT, 2) immediately after the application of KT, 3) after 24h, 4) after 48h, and 5) after 72h. Mixed ANOVA was used to determine differences between groups. Results There was no change in one-footed static balance, electromyographic activity of the VL in the lower limb function, nor in isokinetic performance between groups. Conclusion KT promotes neither immediate nor delayed changes in neuromuscular performance of the femoral quadriceps in healthy women.

Abstract Introduction: Cardiac surgery is a highly complex procedure which generates worsening of lung function and decreased inspiratory muscle strength. The inspiratory muscle training becomes effective for muscle strengthening and can improve functional capacity. Objective: To investigate the effect of inspiratory muscle training on functional capacity submaximal and inspiratory muscle strength in patients undergoing cardiac surgery. Methods: This is a clinical randomized controlled trial with patients undergoing cardiac surgery at Instituto Nobre de Cardiologia. Patients were divided into two groups: control group and training. Preoperatively, were assessed the maximum inspiratory pressure and the distance covered in a 6-minute walk test. From the third postoperative day, the control group was managed according to the routine of the unit while the training group underwent daily protocol of respiratory muscle training until the day of discharge. Results: 50 patients, 27 (54%) males were included, with a mean age of 56.7±13.9 years. After the analysis, the training group had significant increase in maximum inspiratory pressure (69.5±14.9 vs. 83.1±19.1 cmH2O, P=0.0073) and 6-minute walk test (422.4±102.8 vs. 502.4±112.8 m, P=0.0031). Conclusion: We conclude that inspiratory muscle training was effective in improving functional capacity submaximal and inspiratory muscle strength in this sample of patients undergoing cardiac surgery.

RESUMO Objetivo Realizar uma revisão atualizada dos estudos que avaliaram o delirium como fator de risco para o desenvolvimento de demência em idosos. Métodos A revisão foi realizada utilizando-se a base de dados do PubMed, com o seguinte cruzamento de descritores: risk factors and (delirium or acute confusional state) and dementia. Apenas estudos do tipo coorte publicados entre janeiro de 2000 a maio de 2015 foram considerados. Resultados Foram selecionados oito estudos de acordo com os critérios de inclusão e exclusão. Todos os estudos apontaram para um aumento do risco de demência ou declínio cognitivo após a ocorrência de delirium, apesar das diferenças entre os estudos em relação à causa do delirium. Conclusões Os estudos atuais apontam para uma clara associação entre delirium e aumento do risco de demência. Dessa forma, psiquiatras e médicos generalistas precisam ficar atentos aos indícios de declínio cognitivo após a ocorrência do delirium em idosos.


ABSTRACT Objective To perform an update review of the studies that evaluated delirium as a risk factor for developing dementia in older adults. Methods A review was performed using PubMed database, crossing the following descriptors: risk factors and (delirium or acute confusional state) and dementia. Only cohort studies published from January 2000 to May 2015 were considered. Results We selected eight studies according to the inclusion and exclusion criteria. All studies pointed to an increased risk of dementia or cognitive decline after the occurrence of delirium, despite the differences between the studies in relation to the cause of delirium. Conclusions Recent studies indicate a clear association between delirium and increased risk of dementia. Therefore, the psychiatrists and general practitioners need to pay attention to the evidence of cognitive decline after the occurrence of delirium in older adults.

ABSTRACT OBJECTIVE: To test several weaning predictors as determinants of successful extubation after elective cardiac surgery. METHODS: The study was conducted at a tertiary hospital with 100 adult patients undergoing elective cardiac surgery from September to December 2014. We recorded demographic, clinical and surgical data, plus the following predictive indexes: static compliance (Cstat), tidal volume (Vt), respiratory rate (f), f/ Vt ratio, arterial partial oxygen pressure to fraction of inspired oxygen ratio (PaO2/FiO2), and the integrative weaning index (IWI). Extubation was considered successful when there was no need for reintubation within 48 hours. Sensitivity (SE), specificity (SP), positive predictive value (PPV), negative predictive value (NPV), positive likelihood ratio (LR+), and negative likelihood ratio (LR-) were used to evaluate each index. RESULTS: The majority of the patients were male (60%), with mean age of 55.4±14.9 years and low risk of death (62%), according to InsCor. All of the patients were successfully extubated. Tobin Index presented the highest SE (0.99) and LR+ (0.99), followed by IWI (SE=0.98; LR+ =0.98). Other scores, such as SP, NPV and LR-were nullified due to lack of extubation failure. CONCLUSION: All of the weaning predictors tested in this sample of patients submitted to elective cardiac surgery showed high sensitivity, highlighting f/Vt and IWI.

ABSTRACT OBJECTIVE: To evaluate the effects of resistance exercise applied early after coronary artery bypass grafting. METHODS: It is a randomized controlled trial with 34 patients undergoing coronary artery bypass grafting between August 2013 and May 2014. Patients were randomized into two groups by simple draw: a control group (n=17), who received conventional physical therapy and an intervention group (n=17), who received, additionally, resistance exercise. Pulmonary function and functional capacity were evaluated in preoperative period and hospital discharge by spirometry and the six-minute walk test. For statistical analysis, we used the following tests: Shapiro-Wilk, Mann-Whitney, Student'st and Fisher's exact. Variables withP<0.05 were considered significant. RESULTS: Groups were homogeneous in terms of demographic, clinical and surgical variables. Resistance exercise exerted no effect on pulmonary function of intervention group compared to control group. However, intervention group maintained functional capacity at hospital discharge measured by percentage of predict distance in 6MWT (54.122.7% vs. 52.515.5%,P=0.42), while control group had a significant decrease (59.211.1% vs. 50.69.9%, P<0.016). CONCLUSION: Our results indicate that resistance exercise, applied early, may promote maintenance of functional capacity on coronary artery bypass grafting patients, having no impact on pulmonary function when compared to conventional physical therapy.

Fundamentos: Apesar do avanço tecnológico que visa a prolongar a qualidade de vida de pacientes submetidos acirurgia cardíaca, esse procedimento ainda é considerado de alta complexidade. A deambulação precoce é uma alternativa para melhorar a capacidade pulmonar, o condicionamento cardiovascular e o aumento do desempenho funcional. Objetivos: Avaliar o impacto da deambulação precoce sobre o tempo de internação na Unidade de Terapia Intensiva (UTI) e hospitalar em pacientes submetidos a cirurgia cardíaca. Métodos: Estudo transversal, realizado com 49 pacientes submetidos à cirurgia cardíaca e admitidos na UTI, no período de outubro de 2014 a abril de 2015. Os pacientes foram estratificados em dois grupos: com e semdeambulação precoce. Deambulação precoce definida como o ato de caminhar até o terceiro dia de internação na UTI. Análise estatística realizada para verificar a existência de modificações no tempo de permanência hospitalare UTI entre os dois grupos de deambulação.Resultados: Foram estudados 49 pacientes, dos quais 55,1% homens, média de idade 55,2±13,9 anos, internados na UTI em decorrência de cirurgia cardíaca no período do estudo. Não se observou correlação estatística entre o ato de deambular precocemente com o tempo de permanência na UTI cardíaca (3,0±1,5 dias vs. 2,8±1,1 dias, p=0,819) e hospitalar (5,4±3,3 dias vs. 5,3±2,6 dias, p=0,903).Conclusão: A deambulação precoce não se associou a um menor tempo de permanência na UTI ou hospitalar.


Background: Despite the technological advances aimed to extend the quality of life of patients undergoing cardiac surgery, such procedure is still deemed a highly complex intervention. Early ambulation is an alternative to improve lung capacity, cardiovascular fitness and increased functional performance. Objective: Assess the impact of early ambulation on the length of stay in intensive care unit (ICU) and in hospital, for patients undergoing cardiac surgery. Methods: Cross-sectional study of 49 patients undergoing cardiac surgery and admitted to the ICU from October 2014 to April 2015. Patients were stratified into two groups: with and without early ambulation. Early ambulation is the act of walking up to the third day of ICU admission. Statistical analysis performed to check for changes in the length of stay in ICU and in hospital between the two groups of ambulation. Results: The study observed 49 patients (55.1% men) with mean age of 55.2±13.9 years, admitted to the ICU due to cardiac surgery carried out during the study period. No statistical correlation was found between early ambulation and the length of stay in cardiac ICU (3.0±1.5 days vs. 2.8±1.1 days, p=0.819) and in hospital (5.4±3.3 days vs. 5.3±2.6 days, p=0.903).Conclusion: Early ambulation is not related to a shorter length of stay in ICU or in hospital.

AbstractIntroductionAfter removal of endotracheal tube and artificial ventilation, ventilatory support should be continued, offering oxygen supply to ensure an arterial oxygen saturation close to physiological.Objective:The aim of this study was to investigate the effects of positive-end expiratory pressure before extubation on the oxygenation indices of patients undergoing coronary artery bypass grafting.Methods:A randomized clinical trial with seventy-eight patients undergoing coronary artery bypass grafting divided into three groups and ventilated with different positive-end expiratory pressure levels prior to extubation: Group A, 5 cmH2O (n=32); Group B, 8 cmH2O (n=26); and Group C, 10 cmH2O (n=20). Oxygenation index data were obtained from arterial blood gas samples collected at 1, 3, and 6 h after extubation. Patients with chronic pulmonary disease and those who underwent off-pump, emergency, or combined surgeries were excluded. For statistical analysis, we used Shapiro-Wilk, G, Kruskal-Wallis, and analysis of variance tests and set the level of significance at P<0.05.ResultsGroups were homogenous with regard to demographic, clinical, and surgical variables. There were no statistically significant differences between groups in the first 6 h after extubation with regard to oxygenation indices and oxygen therapy utilization.Conclusion:In this sample of patients undergoing coronary artery bypass grafting, the use of different positive-end expiratory pressure levels before extubation did not affect gas exchange or oxygen therapy utilization in the first 6 h after endotracheal tube removal.


ResumoIntrodução:Após a remoção do tubo endotraqueal e ventilação artificial, o suporte ventilatório deve ser continuado, oferecendo suprimento de oxigênio para garantir uma saturação arterial de oxigênio próxima da fisiológica.Objetivo:O objetivo deste estudo foi investigar os efeitos da pressão expiratória positiva final antes de extubação nos índices de oxigenação de pacientes submetidos à cirurgia de revascularização miocárdica.Métodos:Ensaio clínico randomizado com 78 pacientes submetidos à cirurgia de revascularização do miocárdio, divididos em três grupos e ventilados com diferentes níveis de pressão expiratória positiva final antes da extubação: Grupo A, 5 cmH2O (n=32); Grupo B, 8 cm H2O (n=26); e grupo C, 10 cmH2O (n=20). Dados do índice de oxigenação foram obtidos a partir de amostras de gases sanguíneos arteriais coletados em 1, 3 e 6 h após a extubação. Pacientes com doença pulmonar crônica e aqueles que foram submetidos à cirurgia sem circulação extracorpórea, de emergência ou combinadas foram excluídos. Para a análise estatística, foram utilizados Shapiro-Wilk, G, Kruskal-Wallis, e análise dos testes de variância e definição do nível de significância em P<0,05.Resultados:Os grupos foram homogêneos em relação às variáveis ​​demográficas, clínicas e cirúrgicas. Não houve diferenças estatisticamente significativas entre os grupos nas primeiras 6 h após extubação no que diz respeito aos índices de oxigenação e a utilização de oxigenoterapia.Conclusão:Nesta amostra de pacientes submetidos à revascularização do miocárdio, o uso de diferentes níveis de pressão expiratória positiva final antes da extubação não afetou as trocas gasosas ou utilização de oxigenoterapia nas primeiras 6h após a remoção do tubo endotraqueal.

INTRODUCTION: Hypoxemia is a frequent pulmonary complication in the postoperative coronary artery bypass graft. Detection of factors associated with their occurrence may indicate patients at risk for this complication, which allows tracing specific therapeutic and consequently reduce morbidity and mortality. OBJECTIVE: To identify related factors to hypoxemia occurrence in immediate coronary artery bypass graft postoperative. METHODS: In this retrospective cohort study, we studied 100 patients submitted to elective om-pump artery bypass graft , between April 2010 and December 2011, at a reference university hospital for cardiac surgery in the state of Maranhão. It was considered hypoxemia gas exchange ratio less than or equal to 300 mmHg. Associated variables with perioperative hypoxemia were defined by the Student T test, G or Mann-Whitney tests, Chi-square, or Fisher's exact test and multiple linear regression. RESULTS: Among studied variables, high body mass index (P=0.036) and smoking (P=0.024) were significantly associated with hypoxemia in the immediate coronary artery bypass graft postoperative. Hypoxemia incidence in this period was 55% and did not affects mechanical ventilation duration and Intensive Care Unit lengh of stay. CONCLUSION: In this sample, body mass index and smoking were associated to hypoxemia. These data reinforce the importance of clinical assessment to identify patients at risk for this complication, considering its high incidence in immediate postoperative period.


INTRODUÇÃO: A hipoxemia é uma complicação pulmonar frequente no pós-operatório de revascularização miocárdica. A detecção de fatores associados a sua ocorrência pode indicar pacientes de risco para essa complicação, o que possibilita traçar terapêuticas específicas e, consequentemente, diminuir a morbi-mortalidade. OBJETIVO: Identificar fatores relacionados à ocorrência de hipoxemia no pós-operatório imediato de revascularização miocárdica. MÉTODOS: Nesta coorte retrospectiva, foram estudados 100 pacientes que se submeteram à cirurgia eletiva de revascularização miocárdica com uso de circulação extracorpórea, no período de abril de 2010 a dezembro de 2011, no Hospital Universitário de referência para cirurgia cardíaca no estado do Maranhão. Considerou-se hipoxemia índice de troca gasosa menor ou igual a 300 mmHg. A associação das variáveis perioperatórias com a ocorrência de hipoxemia foi definida por meio dos testes t de Student ou Mann Whitney, Qui-quadrado, teste G, ou Exato de Fisher, além de regressão linear múltipla. RESULTADOS: Dentre as variáveis estudadas, o índice de massa corpórea elevado (P=0,036) e o tabagismo (P=0,024) apresentaram associação estatisticamente significante com a ocorrência de hipoxemia no pós-operatório imediato de revascularização miocárdica. A incidência de hipoxemia no pós-operatório imediato foi 55%. A ocorrência de hipoxemia não interferiu nos tempos de ventilação mecânica e de estadia na Unidade de Terapia Intensiva. CONCLUSÃO: Nesta amostra, houve associação entre o índice de massa corpórea e o tabagismo com a ocorrência de hipoxemia. Esses dados reforçam a importância da avaliação clínica para identificação do paciente de risco para ocorrência dessa complicação, já que esta apresenta elevada incidência no pós-operatório imediato.

OBJECTIVE: To compare the effects of different levels of positive end-expiratory pressure on respiratory mechanics and oxygenation indexes in the immediate postoperative period of coronary artery bypass grafting. METHODS: Randomized clinical trial in which 136 patients underwent coronary artery bypass grafting between January 2011 and March 2012 were divided into three groups and admitted to mechanical ventilation with different positive end-expiratory pressure levels: Group A, 5 cmH2O (n=44), Group B, 8 cmH2O (n=47) and Group C, 10 cmH2O (n=45). Data about respiratory mechanics were obtained from mechanical ventilator monitor and oxygenation indexes from arterial blood gas samples, collected twenty minutes after intensive care unit admission. Patients with chronic obstructive pulmonary disease and patients submitted to off-pump, emergency or combined operations were not included. For statistical analysis, we used Kruskal-Wallis, G and Chi-square tests, considering results significant when P<0.05. RESULTS: Groups were homogeneous in terms of demographic, clinical and surgical variables. Patients ventilated with positive end-expiratory pressure of 10 cmH2O (Group C) had best compliance (P=0.04) and airway resistance values, this, however, without statistical significance. They also had best oxygenation indexes, with statistical difference in all analyzed variables, and lower frequency of hypoxemia (P=0.03). CONCLUSION: Higher levels of positive end-expiratory pressure in immediate postoperative period of coronary artery bypass grafting improved pulmonary compliance values and increased oxygenation indexes, resulting in lower frequency of hypoxemia.


OBJETIVO: Comparar os efeitos de diferentes níveis de pressão expiratória positiva final na mecânica respiratória e nos índices de oxigenação no pós-operatório imediato de revascularização do miocárdio. MÉTODOS: Ensaio clínico randomizado no qual 136 pacientes submetidos à revascularização do miocárdio, entre janeiro de 2011 e março de 2012, foram distribuídos em três grupos e admitidos na ventilação mecânica com diferentes níveis de pressão expiratória positiva final: Grupo A, 5 cmH2O (n=44), Grupo B, 8 cmH2O (n=47) e Grupo C, 10 cmH2O (n=45), sendo os dados da mecânica respiratória obtidos do monitor do ventilador mecânico e os índices de oxigenação por meio de gasometria arterial coletada vinte minutos após a admissão na unidade de terapia intensiva. Não foram incluídos pacientes com doença pulmonar obstrutiva crônica, cirurgias associadas, de emergência ou sem circulação extracorpórea. Para análise estatística, empregaram-se os testes de Kruskal-Wallis, Teste G e Qui-quadrado, considerando os resultados significantes quando P<0,05. RESULTADOS: Os grupos apresentaram-se homogêneos em relação às variáveis demográficas, clínicas e cirúrgicas. Os pacientes ventilados com pressão expiratória positiva final de 10 cmH2O (Grupo C) apresentaram os melhores valores de complacência (P=0,04) e de resistência das vias aéreas, esta, porém sem significância estatística. Apresentaram, ainda, os melhores índices de oxigenação, com diferença estatística em todas as variáveis estudadas, além de menor frequência de hipoxemia (P=0,03). CONCLUSÃO: Níveis mais elevados de pressão expiratória positiva final no pós-operatório imediato de revascularização do miocárdio incrementaram os valores de complacência pulmonar e melhoraram os índices de oxigenação, resultando acarretando em menor frequência de hipoxemia.

ABSTRACT

FUNDAMENTO: Síndromes coronarianas agudas são a maior causa de mortalidade no mundo. Estímulos externos, também conhecidos como gatilhos, como estado emocional ou atividade física, podem produzir mudanças fisiopatológicas desencadeantes. Dentre os gatilhos estudados, eventos estressantes, como campeonatos de futebol, são controversos na literatura e não há dados efetivos para a população brasileira. OBJETIVO: Avaliar os efeitos agudos do estresse ambiental induzido pelos jogos da Copa do Mundo de Futebol no aumento da incidência de doenças cardiovasculares no Brasil. MÉTODOS: Foram obtidos dados publicamente disponíveis do Sistema Único de Saúde referentes às internações hospitalares com código internacional de doenças, referentes às síndromes isquêmicas agudas, no período de maio a agosto de 1998 a 2010 (155.992 internações). Restringiu-se a análise aos pacientes maiores que 35 anos e internados por especialidades clínicas. Comparou-se a incidência de infarto e óbito entre os dias sem copa (Grupo I: 144.166; 61,7 ± 12,3 anos; 59,4%masculino), dias de copa sem jogos do Brasil (Grupo II: 9.768; 61,8 ± 12,3 anos; 60,0% masculino) e dias de jogos do Brasil (Grupo III: 2.058; 61,6 ± 12,6 anos; 57,8% masculino). Utilizou-se regressão logística e de Poisson para ajustar por idade, gênero, densidade populacional e número de postos de atendimento. RESULTADOS: Houve aumento da incidência de infarto para jogos de copa do mundo (1,09; IC95% = 1,05-1,15) e do Brasil (1,16; IC95% = 1,06-1,27). Não houve impacto sobre mortalidade - copa (1,00; IC95% = 0,93-1,08) e Brasil (1,04; IC95% = 0,93-1,22). CONCLUSÃO: A copa do mundo e, especialmente, os jogos da seleção brasileira implicam maior incidência de infarto agudo do miocárdio, mas não de mortalidade intra-hospitalar.


BACKGROUND: Acute coronary syndromes are the major cause of death in Brazil and in the world. External stimuli, known also as triggers, such as emotional state and activity, may generate physiopathological changes that can trigger acute coronary syndromes. Among the studied triggers, the impact of stressful events, such as soccer championships, are controversial in literature and there is no effective data on the Brazilian population. OBJECTIVE: To evaluate the acute effects of environmental stress induced by soccer games of the World Soccer Cup on increased incidence of cardiovascular diseases in Brazil. METHODS: Public data were obtained from the Unified Health System (Sistema Único de Saúde), regarding hospital admissions that had the International Code Disease of acute coronary syndromes from May to August, in 1998, 2002, 2006 and 2010 (155,992 admissions). Analysis was restricted to patients older than 35 years and admitted by clinical specialties. The incidence of myocardial infarction, angina and mortality were compared among days without World Cup soccer games (Group I: 144,166; 61.7 ± 12.3; 59.4% males); on days when there were no Brazil's soccer team matches (Group II: 9,768; 61.8±12.3; 60.0% males); and days when there were Brazil's soccer team matches (Group III; 2,058; 61.6±12.6; 57.8% males). Logistic regression was used to adjust to age, gender, population density and number of medical assistance units. RESULTS: The incidence of myocardial infarction increased during the period of World Cup soccer games (1.09; 95%CI = 1.05-1.15) and days when there were Brazil's matches (1.16; 95%CI = 1.06-1.27). There was no impact on mortality during the Cup (1.00; CI95% = 0.93-1.08) and Brazil's matches (1.04; 95%CI = 0.93-1.22). CONCLUSION: World Cup soccer games and, specially, Brazil's matches have an impact on the incidence of myocardial infarction, but not on in-hospital mortality.

ABSTRACT

OBJETIVO: Identificar a prevalência de complicações pulmonares em crianças submetidas à cirurgia cardíaca, assim como características demográficas e clínicas da população estudada. MÉTODOS: A amostra foi composta por 37 crianças, de ambos os sexos, submetidas à cirurgia cardíaca no Hospital Universitário Presidente Dutra, São Luís (MA), durante o ano de 2007. Não foram incluídos pacientes que apresentaram doença pulmonar pré-cirúrgica, portadores de distúrbios neurológicos, óbito intra-operatório, além de falta de dados no prontuário. Os dados foram obtidos pela coleta nas evoluções médicas e de enfermagem dos respectivos prontuários. RESULTADOS: Quanto às características populacionais, houve predomínio de crianças do sexo feminino, provenientes do interior do estado e na faixa etária escolar. Patologias consideradas de baixo risco foram a maioria, destacando-se a persistência do canal arterial, comunicação interventricular e comunicação interatrial. Observou-se que a maior parcela das crianças fez uso de circulação extracorpórea por mais de 30 minutos, sendo a mediana igual a 80 minutos, sofreu esternotomia mediana, utilizou apenas dreno mediastinal e fez uso de ventilação mecânica pós-operatória, sendo a mediana aproximadamente de 6,6 horas. Somente três (8,1 por cento) pacientes apresentaram complicações pulmonares, sendo que destes, dois foram a óbito. CONCLUSÃO: A maioria das crianças da amostra era do sexo feminino, na faixa etária escolar e proveniente do interior do estado. Os baixos tempos de circulação extracorpórea e ventilação mecânica, além de cardiopatias congênitas consideradas de baixo risco, podem ter sido fatores contribuintes para o pequeno índice de complicações pulmonares no pós-operatório.


OBJECTIVE: To identify the prevalence of pulmonary complications in children undergone cardiac surgery, as well as demographic and clinical characteristics of this population. METHODS: The sample comprised 37 children of both genders, underwent cardiac surgery at the Hospital Universitário Presidente Dutra, São Luis (MA) during the year of 2007. There were not included patients who had lung disease in pre-operative period, patients with neurological disorders, intra-operative death besides lack of data in medical records. The data were obtained from general medical and nursing staff of their medical records. RESULTS: The population of the study was predominantly composed by female children, from the countryside and at school age. Pathologies considered low risk were the majority, especially the patent ductus arteriosus, interventricular communication and interatrial communication. It was observed that the largest share of children made use of cardiopulmonary bypass for more than 30 minutes, with a median of 80 minutes, suffered a median sternotomy, using only the mediastinal drain and made use of mechanical ventilation after surgery, with the median about 6.6 hours. Only three (8.1 percent) patients developed pulmonary complications, and of these, two died. CONCLUSION: Most of the sample was female, school aged and from the countryside. The low time of cardiopulmonary bypass and mechanical ventilation, and congenital heart disease with low risk, may have been factors that contributed to the low rate of pulmonary complications postoperative.
